A landmark study published in JAMA Internal Medicine followed 2,315 Finnish men for over 20 years and found that those using a sauna four to seven times per week had a 40% lower risk of all-cause mortality compared to men who used it only once a week. The same cohort, analyzed in Age and Ageing, showed a 66% lower risk of dementia and a 65% lower risk of Alzheimer's disease in the frequent sauna users. A separate analysis published in Neurology found a 60% lower risk of stroke with that same frequency.
Men who used a sauna four to seven times per week had a 66% lower risk of dementia compared to those who used it once a week — a finding that deserves more attention than it gets.
What Cold Exposure Does Inside Your Body
Cold is the other side of this coin, and the mechanism is entirely different. When you step into cold water, your sympathetic nervous system fires. One of the first things that happens is activation of brown adipose tissue — brown fat, the metabolically active kind that actually burns glucose and fatty acids to generate heat. Research published in the Journal of Clinical Investigation showed that just two hours of cold exposure at 16°C significantly increased brown fat metabolic activity and glucose uptake in healthy men. That translates directly to better insulin sensitivity and improved metabolic health.
Cold exposure also appears to reduce chronic low-grade inflammation. A study in PLoS One involving over 3,000 participants found that regular cold showers led to a 29% reduction in self-reported sick days. The mechanism isn't fully mapped, but the working model involves a shift in cytokine balance toward anti-inflammatory signaling with repeated cold adaptation.
Thermal Therapy for Longevity: What This Means for You
The research is observational, and I'll be straight with you: we don't have randomized controlled trials running people through 20-year sauna protocols. But the biological mechanisms are real, the associations are large, and the risk profile for healthy adults is low. Here's how to apply this practically:
Start with heat, build frequency. Aim for 20-minute sauna sessions at 80 to 100°C, working toward four or more sessions per week. If you're new to it, start at five to ten minutes and build gradually.
Hot baths count. If sauna access is limited, research published in the Journal of Applied Physiology showed that daily hot water immersion at 40°C for 60 minutes over eight weeks improved vascular function and reduced arterial stiffness. Two to three sessions per week at 40 to 42°C for 20 to 30 minutes is a reasonable starting target.
Add cold at the end. Finish your shower with 30 to 60 seconds of cold water, working up to two to three minutes over time. For cold plunges, aim for one to five minutes below 10°C, two to four times per week.
Hydrate before and after. Dehydration is the most common mistake with heat therapy. Drink water before you get in and again when you get out.
Know your limits. If you have uncontrolled hypertension, unstable cardiac disease, or you're pregnant, talk to your doctor before starting. These therapies are powerful, and powerful tools require appropriate caution.
