Concierge Medicine: What It Actually Looks Like at Our Practice

Less medications, more answers — and a wellness exam that goes way beyond the basics.

Most people have the same experience at their annual physical. You wait 30 minutes, spend 10 with the doctor, get a basic blood panel, and hear some version of "everything looks fine" or "try to eat better." You leave with more questions than answers.

That's not how we do things at Fort Myers Primary Care & Wellness.

We're a concierge wellness practice, which means our patients pay an annual membership instead of fighting the insurance-driven revolving door. That gives us something most doctors simply don't have — time. Time to listen, time to dig deeper, and time to build a plan that actually fits your life.

What Makes This Different

Dr. Sabha limits his practice to around 300 patients. Compare that to the 2,000 or 3,000 that a typical primary care doctor manages, and you start to see why most appointments feel rushed.

With a smaller panel, every visit runs 30 to 60 minutes. You get same-day or next-day appointments when something comes up. You can call or text Dr. Sabha directly — no phone tree, no waiting three weeks for a callback.

But the real difference isn't just access. It's the approach.

Our philosophy is simple: lifestyle and natural interventions first. We reach for medications only when the evidence clearly supports them — and we actively work to de-prescribe unnecessary ones.

Too many patients come to us on five or six medications when what they really need is the right diet, a structured exercise plan, better sleep, targeted supplements based on actual lab deficiencies, and a doctor who takes the time to connect the dots. That's our starting point.

The Wellness Exam: A Real Deep Dive

Our annual wellness exam runs 90 to 120 minutes, and it combines two things most practices never put together — specialized lab work and hands-on functional testing. The combination gives us a complete picture that neither one provides alone.

On the lab side, we go well beyond standard bloodwork. We check for insulin resistance (the silent driver behind diabetes, weight gain, and brain fog), advanced cardiovascular markers like ApoB particle count and inflammatory markers, thyroid function, liver health, and nutritional status including Vitamin D, B12, magnesium, and homocysteine. These aren't exotic tests — they're just ones that most 15-minute physicals skip.

On the functional side, we measure how your body is actually performing. Body composition analysis reveals your body fat, muscle mass, visceral fat, and metabolic age — going far beyond the bathroom scale. Grip strength testing is one of the strongest predictors of longevity in the medical literature. We screen for silent arterial disease with an ankle-brachial index, assess lung capacity with pulmonary function tests, and evaluate your modifiable dementia risk factors with a 21-point Brain Care Score.

Put it all together and you get something most people have never received from a doctor: a clear, honest picture of where you stand and what to do about it.

Your Personalized Wellness Report

A few weeks after your exam, you receive a detailed wellness report — typically 15 to 20 pages — that ties everything together into a prioritized action plan. It covers your top health findings with real clinical context, a supplement protocol matched to your specific lab results, an exercise prescription tailored to your body composition and fitness level, nutritional guidance based on your metabolic profile, and a monitoring plan with target values and follow-up timelines.

Every recommendation connects directly to your data. Nothing generic. Nothing vague. And because our approach prioritizes lifestyle changes, targeted supplementation, and natural interventions — medications only enter the conversation when they're genuinely needed.

One More Thing Most Practices Can't Offer

Dr. Sabha is also an active hospitalist, which means if you ever need to be hospitalized, your primary care doctor can also be your inpatient doctor. Same physician, same knowledge of your history, seamless continuity. For qualifying conditions, our team can even arrange direct admission — bypassing the ER entirely. It's a small detail until it matters. Then it matters a lot.

What Membership Looks Like

We keep it straightforward — annual membership, no surprise fees. Full-time members 65 and older pay $2,500 per year (about $6.85 a day). Under 65, it's $2,000 per year (about $5.48 a day). Seasonal members pay $1,450 per year — perfect for part-year Southwest Florida residents.

You'll still want health insurance for hospital stays, surgeries, and specialist visits — membership covers the primary care relationship, the advanced testing, the wellness report, and the ongoing access to your doctor. Most members find it pays for itself through better prevention and fewer unnecessary ER visits.
